A pioneer and leading provider of Population Health Management solutions,i2i Systems announced today that the Los Angeles County Department of Health Services (DHS) has selected i2iTracks to support their efforts to provide more proactive care, reduce inappropriate visits to the Emergency Department, achieve Meaningful Use, and implement Patient-Centered Medical Homes and other quality initiatives. i2i Tracks will play a key part in L.A. County DHS’s program to provide accountability, responsibility and transparency in the delivery of patient care.
L.A. County DHS’s extensive provider network includes four hospitals with numerous outpatient clinics, two multi-specialty Ambulatory Care Centers, six comprehensive health centers and multiple health centers throughout the Los Angeles County, many in partnership with private and community-based providers. Close to 400,000 primary-care patients are empaneled. That number could to grow to half a million by the end of the year. Over 800 care providers will use i2iTracks.
L.A. County DHS chose i2i Systems to support quality and patient-centered primary care through Population Health Management. According to Ambulatory Care Network Director of Quality Improvement, Dr. Helen Tran, “quality primary care delivery with accountability, responsibility and transparency is enabled and enhanced through Population Health Management. i2i Systems has a proven track record, extensive community health center experience, and a very large presence in California. Many of our community partners, including non-county clinics, use i2i Systems as their Health Population Management solution.”
The i2i Systems approach to Population Health Management combines a philosophy dedicated to 100% proactive care with a robust platform of smart tools that enable healthcare providers to act and measure the work required to deliver and manage optimum care. Population Health Management, however, extends beyond being effective and efficient at the point of care; it’s also about knowing the health of the population of patients not in front of you, but may require outreach. Interfacing with L.A. County’s data warehouse and the patient management systems, i2i Systems allows L.A. County DHS to track the health of the populations served – for example, the average hemoglobin A1C in their diabetic population, or compliance with preventive services such as mammograms and pap smears in women over 40. In addition, i2iTracks enables L.A. County DHS to generate extensive metrics on the level of care provided.
Extending beyond the ambulatory primary care setting, L.A. County will use i2iTracks to manage care in the specialty medical homes such as HIV/AIDS. Rancho Los Amigos National Rehabilitation Center, nationally ranked for excellence, will use i2iTracks to help support care for stroke, spinal cord injury and amputation specialty medical homes.
