As a tribal Medical Clinic, Greenville Rancheria is part of Indian Health Services (IHS), under the purview of the Bureau of Indian Affairs. Greenville Rancheria serves their CHDSA areas of Plumas and Tehama County. Originally set up to serve the Plumas County Maidu Tribes, Greenville Rancheria has evolved from serving the tribal population, to the geographic region. Originally a general primary care family practice, it now offers multiple sub-specialties, including cardiology, psychiatry, pain management, orthopedics, rheumatology, dental physical therapy, ENT, OB/GYN and Internal Medicine. In the last three years, Greenville Rancheria has grown from a small clinic to three facilities and 27 providers, with more expansion on the way.

“Growth and expansion of services offer both opportunities and challenges,” explains Greenville Rancheria’s IT Supervisor Scott Harding. “Under Indian Health Services, our reporting and grant writing needs are quite varied, complex and extensive. Our electronic health record system was effectively meeting our individual patient, point-of-care needs; however, we found it too labor intensive, too time-consuming to pull the data for required reports. We needed a reporting solution that was flexible and would not tax our internal resources. We needed a population health management solution.”

Greenville Rancheria chose i2iTracks Population Health Management solution to support its demand for generating a wide range of reports. According to Harding, “i2i Systems came highly recommended by several other tribal health systems as someone who can produce reliable reports quickly and easily. We apply for many grants, all having unique data requirements. These grants often ask for non-conformed data on different populations within the communities we serve. Because we can identify, organize, sort and pull specific data, we can, for example, easily identify, outreach and monitor our diabetic patients’ HbA1c. We can also use i2iTracks to document conditions related to diabetes – obesity and hypertension, for example. The power of i2iTracks is that it helps us connect and document all these conditions, which promotes better care. And even with our expansion, i2i Systems provides a solution where reporting doesn’t take us weeks but just hours to produce. This is especially important as Greenville Rancheria seeks to qualify for full FQHC status, which involves a new set of data requirements.”
