Cloud-based Integrated Case Management Complexity Assessment Grid Now Launches Directly from EssetteCM™ Case Management Application Suite

In a collaborative effort, the Case Management Society of America (CMSA) has teamed up with Essette to launch the cloud-based Integrated Case Management Complexity Assessment Grid (ICM-CAG) directly from the EssetteCM™ case management application suite. The interface allows a user/case manager, who has successfully completed ICM-CAG training, to access the ICM-CAG questionnaire grid with a single click from the Case Acuity function in the EssetteCM™ module.

The ICM assessment process fosters the development of an alliance between the case manager and patient/client using motivational interviewing skills while disentangling physical, psychological, social, and health system barriers to improvement. Developed in Europe by the INTERMED foundation, the complexity score for a patient, when assessed in a consistent manner, has been validated in studies as a predictor of future health care utilization.

ICM training is an advanced educational program designed to provide case managers coming from either medical or mental health backgrounds with the content and skills needed to perform systematic complexity assessments; to develop and carry out comprehensive and coordinated physical and mental health care plans; to apply consistent complexity-based anchor points in specially designed ICM clinical documentation software; and to record ongoing clinical, functional, quality of life, satisfaction, and fiscal outcomes during the management process.

About ICM Training & CMSA
Training in ICM has been in development since 2008 by the Case Management Society of America and Cartesian Solutions, Inc.™ It is based both on the application of 15 years of complexity research largely performed in Europe and also on CMSA’s 2010 Standards of Practice for Case Management(http://www.cmsa.org/sop). To date, nearly four hundred case managers in the United States have participated in official training sessions.
