HealthyCircles launches the new Mobile Application for iPhones and iPads to help health care professionals better engage with their patients and family caregivers, offering mobile care management without the hassle of additional equipment fees or servicing.

The addition of the mobile application will greatly impact patient engagement efforts by connecting them to their care by reaching them where they are.

Today, HealthyCircles™ (http://www.healthycircles.com), a private-label enterprise solution designed for integrated care coordination and management, officially announced the launch of its mobile application, allowing for increased care coordination and patient engagement.

The new HealthyCircles mobile application is more than just a mobile app as it provides patients and family caregivers the opportunity to stay engaged and is linked to the HealthyCircles Consumer Portal, allowing for a more flexible experience that fits their busy lives. This mobile app will allow patients to respond to care management programs remotely and display responses graphically on the health care professional’s care management dashboard while also allowing all Care Team members to remotely communicate.
