Pathways Home Health, Hospice & Private Duty is celebrating its 35th anniversary of providing care for patients and families this month. Pathways was founded in 1977 by several physicians who saw a community need for compassionate end-of-life care. The not-for-profit healthcare agency was originally known as Mid-Peninsula Health Services but was renamed Pathways Home Health, Hospice & Private Duty when it began to serve patients beyond the peninsula area.
During its first year, Pathways cared for 12 patients. In 2011, Pathways cared for over 5,000 home health, hospice and private duty patients in a service area that ranges from San Francisco to Gilroy to San Pablo from offices in Sunnyvale, Oakland, and South San Francisco.

Pathways’ mission is to provide compassionate, high quality patient and family-centered home health and hospice care, promoting comfort, independent and dignity. Over the years more than 2,000 physicians in the Bay Area have entrusted their patients to Pathways.
Pathways is unique in the Bay Area in that we have three service lines: a Hospice for those with life-limiting illness; a Home Health program to care for people after surgery or with wounds or chronic illness; and Private Duty to make caregivers available to those who want to hire help at home. The agency’s programs include IV therapy; Pathways KIDS for children with life-limiting illness; Palliative Care; specialized cardiac care; PATH: Pharmacy Assisted Transition to Home; wound care; rehabilitation; HARP: Housecall Admission Reduction Program to reduce the need to be re-admitted to a hospital; and integrative therapies holistic care for hospice patients.

Donations received from the community by Pathways Home Health & Hospice provide services that would otherwise not be available, including 24/7 access to a specialized team of care providers, chronic care programs, family bereavement counseling and support, integrative therapies, caregiver support, comfort care, and funding for under- and uninsured patients.
