Over 9% of the adults in Burke County have been diagnosed with diabetes and an additional 8% have undiagnosed diabetes or are considered pre diabetic, according to a study by the State Center for Health Statistics, DHHS. Additionally, the study reported that in 2008 (most recent statistics available), over 13,000 people in Burke County were estimated to suffered from diabetes or pre diabetic conditions.
Responding to this tremendous need, Burke Primary Care of Morganton implemented a diabetes self-management education program that recently received the American Diabetes Association’s prestigious Education Recognition Certificate.
The ADA certificate recognizes the family practice for high-quality patient education that is essential to effective diabetes treatment. The certificate also affirms that the staff administering the program is composed of knowledgeable health professionals who provide participants with comprehensive information about diabetes management.
The diabetes self-education program at Burke Primary Care is facilitated by Tara Mode, a Registered Nurse and Certified Diabetes Educator. “I was diagnosed with Type 1 Diabetes 20 years ago and I know how challenging it is to learn how to control diabetes to maintain a healthy, normal life,” she said. Mode has over 14 years experience in diabetes education.
Mode continued, “I work closely with the patient’s primary care provider to develop the best possible diabetes management plan for the patient. We give our patients the resources and education they need to successfully manage their diabetes.”
The diabetes education program also helps the families of diabetic patients. “Understanding the disease is key to helping our patients control their diabetes. Once the family is supportive and understands the difficulties of life with diabetes, the diabetic patient can have a healthier lifestyle and handles their diabetes management more successfully,” said Mode.
