Sound Shore Health System Hospitals – Sound Shore Medical Center and Mount Vernon Hospital, are pleased to be founding members of and provider hospitals for Hudson Valley Care Coalition (HVCC) led by Open Door Family Medical Centers – one of only three provider organizations selected by the New York State Department of Health to function as a Medicaid Health Home in the Hudson Valley. The collaborative effort started last year in Westchester by Hudson Health Plan and now encompasses countless organizations in six counties in the Hudson Valley region.

Today, HVCC is an impressive network of institutions and providers working together to improve healthcare for a segment of Medicaid recipients with multiple chronic conditions – or at risk for developing more than one. As a Medicaid Health Home, HVCC delivers leading‐edge care, functions as a Think Tank and engages individuals with serious medical and behavioral health issues by addressing their health needs as well as their social and environmental concerns – such as food and shelter. This is accomplished through comprehensive care management, health promotion, transition care, patient and family support, referral to community and social support services and linking services through Health Information Technology. With the goal of engaging the population to stay in care, access services needed and manage diseases, HVCC is on track to lessen the drain on healthcare resources and reduce healthcare costs.
