This partnership will help families of HCA Virginia patients navigate the discharge process and connect with service providers and resources that can support them with their caregiving responsibilities once they leave the hospital.

As an online resource serving family caregivers, Liftcaregiving.com focuses on providing support and information to individuals trying to navigate the often confusing role of caregiver to a family member. The website offers articles written by experts, a caregiver message board and proven products that can help with daily life. The site also features helpful online tools such as Family Ties, a free care management tool, developed to help caregivers manage the care of their loved one and a database of local and national service providers (https://www.liftcaregiving.com/local-resources/).

Decisions at Discharge Planning Guide: This tool helps family caregivers identify and connect with service providers such as in-home nursing care and meal delivery services in their area.

Care Lounge Community: This virtual community helps family caregivers connect with professionals and other caregivers to ask questions, engage in discussions, seek best practices and alleviate the isolation that sometimes comes with caring for a family member.

The Advice Library: This suite of articles written by physicians, caregivers and caregiving professionals helps family caregivers learn more and get tips for caring for an aging or ailing family member.
