Liposuction and tummy tuck repair defects (like "dog ears" at the end of the incision) are usually caused by aggressive and/or uneven removal of fat from the body, by uneven healing and scar tissue formation, or changes in weight (usually weight gain) after the procedure. After surgery, there often appear peaks and valleys in areas of protruding fat that remains behind, or hollow hail damage-like indentations where too much fat was extracted. Correcting these areas is usually difficult because the fat is trapped and encased in old scar tissue and scar tissue is pulling the skin inward at the "dents."

VASER is the trade name for an ultrasound-assisted liposuction system that uses sound waves to selectively emulsify (liquefy) fat below the skin. More importantly, Vaser can be used to break through the scar and free up the trapped fat to get a smoother appearance. Liquefying (melting) the fat allows for easier removal or shifting of the fat with thin, small, less painful instruments. This kinder, gentler form of sculpting the unsightly fatty areas of the body is done as an office procedure with the patient wide awake, with no general anesthesia – saving thousands of dollars in hospital and anesthesiologist costs.

The number of patients experiencing undesirable post liposuction results is on the rise. With cosmetic enhancement becoming more popular and affordable, liposuction has become the number one aesthetic plastic surgery procedure. As with anything else, it is always better to get good results the first time. However, bad liposuction results can be improved in most cases, where there was little hope for improvement.

Correction requires a combination of loosening scar tissue, fat removal, fat shifting, or fat transfer (fat transplant) techniques. Liposculpture removes excess fat in the “peaks” of problem areas, making the area smoother and creating a more even surface. For the “valleys”, fat is harvested from another part of the body and injected into the depressed area to raise it to the same level as the surrounding skin.

When the “peaks and valleys” are fairly close together, correction is performed using lipo-shifting. After disrupting the tight scar tissue, fat is shifted from the “hilly” area to the “valley,” under the skin. Most of the time the correction is carried out through the previous incision marks, to avoid creating new scars. Anti-inflammatory supplements are recommended to minimize formation of new scar tissue under the skin during healing.

Patients are advised to wear girdle-like medical compression garments, under the clothes, to hold the skin and repaired area flat and smooth during the healing phase. Downtime after this procedure is minimal: most patients are driving or working after two to four days, without the need of strong narcotic medications, since discomfort, swelling, and bruising are less than with the older tumescent liposuction operations. Complications can occur, but are rare when the procedure is done by a qualified surgeon and the patient strictly follows pre- and post-operative instructions. Results of ultrasonic liposuction correction and revision can be permanent as long as transplanted fat cells survive and there is minimal formation of recurrent scar. A touch-up may be required in some cases. Maintaining a healthy weight and lifestyle optimizes results.
