Sobrato Family Foundation Awards Grant to Pathways Home Health & Hospice to Support Expanded Services

Pathways Home Health & Hospice, a home health, palliative care and hospice agency, received a two-year $123,000 challenge grant from the Sobrato Family Foundation to help support the growing demand for home health, telemonitoring, chronic care, palliative, and hospice programs.

The Sobrato Family Foundation is a recognized leader in philanthropy in the Santa Clara Valley, Their continued support of Pathways Home Health & Hospice is a vote of confidence.

Pathways Home Health & Hospice, a home health, palliative care and hospice agency, received a two-year $123,000 challenge grant from the Sobrato Family Foundation. Pathways will use this grant in part to make renovations to their headquarters in Sunnyvale to accommodate the increase in staff needed to manage and support the growing demand for our home health, telemonitoring, palliative, and hospice programs.

Pathways has received the first installment of $60,000 which must be matched by new or increased donations by December 31, 2013. Due to this grant, individuals and organizations can now double the impact of their donations to Pathways when their giving this year exceeds that of last year. Pathways was founded in 1977 by a group of Stanford physicians and community members who saw the need for professional care at home. That first year, Pathways served 12 patients.
