Release 4.6 focuses on TruCare’s newest feature, Home and Community Services, which facilitates reducing the costs per member associated with acute and long-term care while also improving the member’s quality of life.

Forty-eight of fifty states support Home and Community Service programs that manage, trace, approve, and regulate costs for non-health care related services that are vital to maintain the quality of life of often extremely sick individuals.

Home and Community Services utilizes TruCare’s Differential Care Management framework while also drawing on the existing Case Management and Utilization Management structure, flexibility, and functionality such as Tasks, Notes, Correspondence, and Key Metrics. Components of this new feature include:

  •     Team Based Assignments
  •     Member Centered Plans
  •     Individual Service Plans
  •     Service Requests
  •     Non-Medical Provider

This past June, Casenet announced its 4.5 release of TruCare which introduced Transportation Management; a sub system which helps authorize, schedule, manage, and maintain ride requests for managed care members. Casenet’s Home and Community Services subsystem along with Transportation Management, positions the TruCare product to be particularly well suited for use by health plans and other entities that manage the high acuity population.
