The technology will enable providers at Beaufort Memorial to more effectively coordinate and manage care for improved patient and population health.
The hospital is currently using the EMPOWER enterprise patient portal and PREDICT customer relationship management system to engage patients and families in their health outside the hospital walls. With the NAVIGATE solution, Beaufort Memorial will have technology to better provide coordinated care management, personalized to low-, rising-, and high-risk patients.
