My Health Medical Group Named 2013 Patient-Centered Medical Home of the Year, Becomes First in San Francisco to Earn Top NCQA Recognition

My Health Medical Group, an affiliate of Brown & Toland Physicians (, won the 2013 “Patient-Centered Medical Home Practice of the Year” award from the California Academy of Family Physicians at its annual conference March 2 and 3.

My Health Medical Group ( also became the first practice in San Francisco to earn recognition as a “Level 3 Patient-Centered Medical Home” from the National Committee for Quality Assurance (NCQA). Level 3 is the highest recognition possible from NCQA, a nonprofit healthcare accreditation entity that validates practices as patient-centered medical homes using a rigorous set of national industry standards.

“We are surprised and delighted by the recognition we’ve received so far,” said Lawrence Shore, M.D., lead physician at My Health Medical Group. “I feel these achievements help validate the idea that we are truly on our way to transforming the delivery of primary care through the patient-centered medical home model. The NCQA recognition and the Academy’s support give all of us at My Health Medical Group some extra momentum to continue improving the healthcare experience for all our patients.”

At My Health Medical Group, health care teams including physicians, nurse practitioners, medical assistants, data analysts, and other staff work together to provide seamless support to patients. The practice also offers patients on-site laboratory services, same-day appointments, extended hours, and the ability to communicate securely with their physician using an online patient portal.

This collaborative approach to primary care helps foster a more developed relationship between the healthcare team and the patient, and improves patient engagement. The end result: greater patient satisfaction as well as improved clinical outcomes.
