Maui County Office on Aging Selects Harmony Information Systems for Care Transitions

(Harmony), the leader in integrated health and human services management, announced today that the Maui County Office on Aging has selected the Harmony for Care Transitions solution to manage the Community-based Care Transitions Program (CCTP) improving care transitions from the hospital to other settings and reducing readmissions for high-risk Medicare beneficiaries.

The CCTP was launched in 2011 as part of the Affordable Care Act to improve transitions of Medicare beneficiaries from inpatient hospital setting to other care settings, reduce readmissions for high risk beneficiaries, and document measurable savings to the Medicare program. Under this program, 102 community-based organizations (CBOs) will use care transition services to effectively manage Medicare patients' transitions and improve their quality of care.
