The US Air Force uses the form 910 to record the performance of Air force members over their number of years in service with the force. The AF 990 form is also called Enlisted Performance Report. Once the evaluation process is over, the superior officer of the concerned will put a grade on his Form 910. The rating officer is free to provide some confidential report on the applicant’s overall performance.
