Hairline lowering, on which Dr. Kabaker is an expert, is the surgical act of bringing the hairline of a patient lower to reduce the size of the forehead. When men have a high hairline it is usually due to progressive male pattern baldness. Performing an average of 60 hairline lowering procedures each year, he has been at the forefront of the evolution of forehead reduction methods. New, better techniques produce consistent excellent results for Dr. Kabaker’s patients. For instance, Dr. Kabaker has stopped creating multi saw-toothed incisions lines, in exchange for undulating patterns in the hairline. Also, after galeotomy incisions (shallow incisions on the undersurface of the scalp) are made, a new stretching technique is regularly employed to get further forward movement of the scalp. And, in many cases, a change of the hairline shape can be achieved in addition to it being lowered.
Other changes have led to improved outcomes which give undetectable scarring. Incisions are never placed in front of the temporal hairline, as those scars can be difficult to disguise. When suturing the skin back together, the skin closure is now performed to include more of the forehead skin vs. the scalp skin so hair is better able to grow through and in front of the scars, acting as camouflage. Patients who need narrowing of the forehead in addition to it being shortened can have a hair transplant session done in the same operation without making additional incisions. These grafts (up to 650 in a case) are placed in front of the temple hair on select occasions. Dr. Kabaker warns against using these grafts in the scar or in the front of the hairline. Grafts can certainly be done later but in have rarely been wanted or done for Dr. Kabaker’s patients.
