Seattle Business Magazine Selects Clarity Health as Finalist for Technology Impact Award
Seattle (PRWEB) September 10, 2013 -- Clarity Health Services, a rapidly-growing provider of care coordination solutions for medical practices and health systems, was honored as a finalist for Seattle Business Magazine’s Tech Impact Award in the health category. The Tech Impact Awards recognize companies and individuals that create or use information technology to significantly improve a company or industry’s business performance or to provide an important public benefit.
Clarity Health was honored specifically for its Emergency Department (ED) Discharge Notification product, a web-based system that gives healthcare providers the real-time information they need to make sure vulnerable patient populations receive appropriate and timely follow-up care when they leave the hospital. This helps to improve patient outcomes and reduces the need for re-admission.
Clarity Health customers, such as Summit View Clinic in Puyallup, Wash., have seen a 10 percent reduction in re-admission rates almost immediately after deploying Clarity without substantial investment in new resources or complex new processes. Simply through the enablement of real-time, web-based notifications, patient outcomes have improved.
“In our busy clinic, the real-time tracking on Clarity’s ED Notification allows us to be proactive with our patients’ care,” said Pam Bluford, RN, care coordinator at Summit View Clinic. “In addition to seeing who’s been in the emergency room, I can see which patients are there right now so we can take action quickly. The faster we act, particularly with our high-risk patients, the better our quality of care.”
Clarity Health’s ED Discharge Notification is part of a broader platform of patient transition services that includes referral management and surgery authorization to increase efficiency in medical settings and ultimately increase patient satisfaction.
“Active patient follow-up is absolutely critical following an emergency department visit,” said Clarity Health CEO Bill Bunker. “All too often, however, patients are left to fend for themselves when they leave the emergency department. Clarity’s solution closes this gap so patients can rapidly get the effective follow-up care they need. It’s gratifying to see our solution recognized not only by Seattle Business Magazine, but by the primary care physicians who are already using our tools to improve care outcomes.”
