TransforMED Celebrates Membership Milestone for Delta-Exchange Learning Community with New Social Analytics, Topical Health Care Information Technology Zone
San Diego, CA (PRWEB) September 26, 2013 -- Delta-Exchange, an award-winning online learning community for primary care practice transformation powered by TransforMED, a subsidiary of the American Academy of Family Physicians, is rolling out a social analytics and member engagement tool as well as a Health Information Technology Zone as its membership nears 10,000.
Launched in 2009, Delta-Exchange gives primary care physicians and their staff the opportunity to communicate, collaborate and learn from other practices as they seek to overcome the challenges of implementing new technologies and workflows that are key to patient-centered medical home and medical neighborhood transformation. Delta-Exchange addresses these needs in an online, social networking platform.
At the 2013 AAFP Scientific Assembly, held Sept. 24-28 in San Diego, TransforMED will roll out a new social analytics and member engagement tool, and an HIT Zone devoted to the specific topic of HIT and electronic health records. Both enhancements were created based on member feedback and surveys to add value to the rapidly growing private community.
New Delta-Exchange social analytics will help hospital systems, payers and practices track overall member engagement, trending content, key change drivers and more. Preconfigured dashboards will display weekly data and offer reporting at community, space, group and individual levels. Delta-Exchange social analytics will be powered by Washington-based MicroStrategy, a leading global provider of business intelligence, mobile intelligence and social intelligence applications that was recognized as a “leader” in the 2013 Gartner Magic Quadrant for BI and Analytics Platforms.
“Four years ago, we launched Delta-Exchange with 18 members, and to see it close in on 10,000 members is simply exciting,” said Nathan Bieck, marketing and communications director at TransforMED. “In our workings with health systems, the federal government and payers specifically, having the ability to analyze practice engagement by day, week or month is critical to transformation and to the success of the project. Understanding the overall engagement within the learning community will allow us to adjust and focus in on specific elements of transformation.”
The new HIT Zone on Delta-Exchange is a specific topical zone devoted to questions, meaningful discussion and sharing around leading EHRs, registries and patient portals being used today. The goal is to provide primary care practices transforming to a PCMH or PCMN with one place to access the tools, experts, peer support and HIT services they need in order to achieve the triple aim of better health and better care at lower costs.
"As new prospects emerge for primary care physicians to become more engaged in health system and payment reform, it is crucial for them to be ready when opportunity knocks," said Bruce Bagley, MD, FAAFP, interim president and CEO of TransforMED.
"In today's changing health care environment, it is imperative that primary care doctors be armed with strategies, peer-to-peer connections and meaningful information to optimize patient outcomes and lower costs,” he said. “Delta-Exchange is an excellent starting point for all primary care physicians who are looking to make medical home and medical neighborhood changes within their practice and community.”
“One of the biggest challenges I face as a solo family physician is gaining access to the experts and resources necessary to transform my practice to a medical home,” said Delta-Exchange member William Bailey, MD. “Delta-Exchange provides the tools I need, when I need them, and is very user-friendly. Delta-Exchange connects me with other physicians from across the country to share challenges, experiences and successes. The site also has put me in touch with experts in the field of patient-centered medical home transformation.”
