ABQAURP Announces New Multimedia Online Course: Transitions of Care and Reducing Readmissions Boot Camp
New Port Richey, FL (PRWEB) September 27, 2013 -- Nearly one in five Medicare patients is readmitted within a month, which the government considers a prime symptom of an overly expensive and uncoordinated system. It is clear that CMS, and intuitively other payers of health care, are aggressively looking at every opportunity to improve efficiencies and reduce waste in all transactions of care across the health care continuum.
The “Transitions of Care and Reducing Readmissions Boot Camp” course was devised to provide participants an understanding of the rules for Medicare's Hospital Readmission Reduction Program.
This course carries 8 hours of continuing medical education hours and 8 hours of nursing continuing education. The course focuses on:
• Providing an overview and update of national strategies addressing transitions of care and reduction of readmissions
• Discussing specific roles physicians and their extenders can play to reduce inappropriate readmissions
• Discovering Transitions of Care opportunities to reduce readmissions in high risk patients
• Exploring initiatives that have been successfully employed to reduce readmissions in hospitals
