Nearly one in five Medicare patients is readmitted within a month, which the government considers a prime symptom of an overly expensive and uncoordinated system. It is clear that CMS, and intuitively other payers of health care, are aggressively looking at every opportunity to improve efficiencies and reduce waste in all transactions of care across the health care continuum.
The “Transitions of Care and Reducing Readmissions Boot Camp” course was devised to provide participants an understanding of the rules for Medicare's Hospital Readmission Reduction Program.
This course carries 8 hours of continuing medical education hours and 8 hours of nursing continuing education. The course focuses on:
- Providing an overview and update of national strategies addressing transitions of care and reduction of readmissions
- Discussing specific roles physicians and their extenders can play to reduce inappropriate readmissions
- Discovering Transitions of Care opportunities to reduce readmissions in high risk patients
- Exploring initiatives that have been successfully employed to reduce readmissions in hospitals
“The speakers at this conference were true thought leaders and provided a clear picture of where health care is going, what is being assessed and planned for, and how the ‘new patient (as a consumer of healthcare)’ will impact those that do not accept change,” said Terri Street RN BS, Director of Specialty Programs at Home Solutions. “I was able to strengthen our home infusion transition of care and patient outcome monitoring program to embrace the change and help our referral sources. I strongly recommend attendance and I am thankful to work for a company that is supporting my involvement,” she continued.
David G. Schulke, Vice President, Research Programs, Health Research and Educational Trust (HRET) / American Hospital Association spoke of the content by saying: “The ABQAURP conference focused on a timely topic, hospital readmissions, and brought leading speakers who have recently been published and who are shaping this field. I learned a great deal of information that I have already found useful in my work. Kudos to ABQAURP!”
