The event is scheduled for Monday, March 3, 2014 at 3:00 p.m. ET.
During this webinar, Dr. Coleman will offer his perspective on the challenges that aging services organizations commonly face when implementing care transitions programs, including collaborating with hospitals, managing costs, establishing effective business processes, and more. He will also provide guidance for aging services organizations to overcome these challenges and optimize resources to increase the success rate of transitions for their consumers.
The Community Care Transitions Program (CCTP) was launched in 2011 as part of the Affordable Care Act to improve transitions of Medicare beneficiaries from inpatient hospital setting to other care settings, reduce readmissions for high risk beneficiaries, and document measurable savings to the Medicare program. Under this program, 102 community-based organizations (CBOs) will use care transition services to effectively manage Medicare patients' transitions and improve their quality of care.
Harmony for Care Transitions enables CBOs and hospitals to secure available Medicare program funds and to collaborate, create, plan, and deliver high-quality transitional care. With Harmony, CBOs are able to create transition care plans and manage and track transitions in coordination with hospitals.
