Vonlay, LLC, a full-service healthcare consulting firm, announced they are working with UW Health to aggregate population management data and improve healthcare delivery for their patients.
Phase one of the project is the establishment of patient registries. Vonlay will work with UW Health (made up of University of Wisconsin Medical Foundation and UW Hospitals and Clinics) to identify patients with common attributes: for example, patients with hypertension, diabetes or asthma. The next step will be developing electronic medical record (EMR) tools, such as outreach encounters and care plans, to manage populations for better outcomes.
UW Health is reinvesting funds received from their successful completion of Meaningful Use stage one to support this effort. Completion is scheduled for late 2014. The work from this project will expand functionality available to UW patients and providers and contribute toward the organization’s efforts to meet Meaningful Use stage two requirements. The organization uses Epic for its EMR.
As patient registries are established, Vonlay will develop reporting and outreach tools to offer patients with chronic conditions preventative medical care. There will also be emphasis on designing a strategy to better capture and react to patient-reported outcomes.
One major deliverable will be the implementation of a provider dashboard. This tool will allow staff to identify commonalities in patients with similar diagnoses, run reports to see whether patients that fit criteria need specific tests, easily send questionnaires through MyChart, manage patient reported outcomes, and more.
An effort to launch more robust care plans is also within project scope. An effective care plan assimilates all patient documentation and insights and ensures communication across the entire care team.
