i2iTracks has really helped propel us forward to the next level in value-based health care,” said Shawn Crawford, CEO of CVFP
Santa Rosa, CA (PRWEB) April 02, 2014
A pioneer and leading provider of Population Health Management (PHM) solutions, i2i Systems, announced today that Central Virginia Family Physicians (CVFP) has selected i2iTracks to support them in achieving various value-based care initiatives to improve population health and reduce per capita costs. As they advance to Level 3 certification for Patient- Centered Medical Home (PCMH) this July, CVFP is using i2iTracks to improve coordination of care, streamline care team workflows, and improve outcomes of care across their 11 practice sites and in collaboration with local medical facilities and providers.
As one of the founding partners of the Collaborative Health Partners of Virginia --an integrated network of primary care physicians, including Medical Associates of Central Virginia – CVFP is working toward improving and enhancing healthcare delivery throughout the region to deliver the highest quality care at the lowest possible cost. The efficient 70-day deployment of i2iTracks has already resulted in care team efficiencies, improved EHR documentation and proactive care outreach. “i2iTracks has really helped propel us forward to the next level in value-based health care,” said Shawn Crawford, CEO of CVFP.
One of CVFP’s top priorities is the redesign of our care teams including the addition of health navigators that coordinate care and improve patient engagement. “We wanted our health navigators to have the right tools to proactively manage our patient’s health and to engage in outreach for important preventive care, i2iTracks will be critical in supporting the health navigators.” said Crawford.
“i2i has helped us develop platforms that allow us to measure and improve the quality measures to provide better patient care in our medical home,” said Dr. Robert Elliott, CVFP Physician, Clinical Assistant Professor Family Medicine University of Virginia Health Systems.
Another priority has been EHR documentation. CVFP chose i2iTracks for its ability to provide extensive insight into their EHR data to uncover data quality issues. Through the use of i2iTracks, CVFP has improved EHR documentation and now has reports with more accurate data to support care teams in coordination of care, including lab and referral tracking, management of subpopulations, and tracking hospital admissions and discharges.
“i2iTracks has helped us standardize our care processes,” said Crawford. “In addition to streamlining diabetic patient management, we are also looking at other risk factors, such as hypertension and smoking cessation, so we can reengage patients who need to be seen. i2iTracks gives us greater capability to connect with patients and make sure they are really engaged in their own care.”
As CVFP works toward PCMH transformation, i2iTracks has provided a systematic approach for implementing policies and protocols, reporting on clinical quality measures (CQMs), and transforming care teams to help their first site achieve Level 3 PCMH certified this July. “Without i2iTracks, we would have to do all our reporting and data capture manually, which would require more resources that we simply do not have,” said Crawford. “i2iTracks has really helped us expedite the process.” Their goal is to have their remaining 10 sites PCMH certified by the end of 2014.
“We believe that the future of medicine is integrated care that is centered and focused on proven quality, cost efficient primary care. This not only a local vision but one that reaches every corner of our country. i2i has is an important component in reaching that goal.” Dr. Thomas Eppes, President of Central Virginia Family Physicians.
The early impact of i2iTracks has not only been care team efficiencies. As a result of improved population health management, CVFP also has the ability to earn additional payments from payers for high performance. The analytics capability and outcomes reporting in i2iTracks is helping them strategically reengage both payers and employers to find ways to fill the gaps in care. With the support of i2iTracks, CVFP will continue to reach out to and partner with other primary care and physician practices within the community to deliver high-quality care at a low cost.
