Pipe Trades Services of Minnesota Consummates Population Health Management Agreement With U.S. Preventive Medicine

(USPM) is pleased to announce the Pipe Trades Services of Minnesota (PTSMN) has contracted USPM to provide The Prevention Plan™, The Prevention Plan Care Management™, and 24/7 nurse triage service at the various primary care clinics open to serving PTSMN union members.
PTSMN represents 6,000 Plumbers, Pipefitters, Sprinklerfitters, Gasfitters, and HVAC Service Technicians throughout the state of Minnesota with members working in all areas of the plumbing, pipefitting, mechanical and fire protection industry, including residential, commercial and institutional facilities.

USPM’s exclusive Prevention Score is the health status equivalent of a credit score relating to financial status; the HRA, biometrics results, participation in nutrition and exercise activities, health challenges, educational programs, and health coaching, all aggregate up to improve each member’s Prevention Score.
USPM also offers Care Management services employing specialized nurses who work directly with participants to manage chronic conditions such as Diabetes, COPD, Asthma, Hypertension and Coronary Artery Disease. As an extension of the member’s physician-directed treatment plans, USPM nurses provide Prevention Plan members with a specific roadmap to prevention to help them proactively participate in the process of improving their wellbeing while gaining a better understanding of their overall health.
PTSMN is a longtime member of the Labor Management Health Care Coalition of the Upper Midwest (LMHCC) and is pleased to be utilizing another of the coalitions programs.
