VNA Health Group Presents on Community-Based Care Transitions Program at New Jersey Hospital Association Conference
(PRWEB) May 08, 2014 -- VNA Health Group, the lead agency for the Central New Jersey Care Transitions Program (CNJCTP), is presenting at the New Jersey Hospital Association’s Readmissions and Improving Transitions of Care conference on May 8, 2014. The CNJCTP is a collaborative of multiple hospitals and community organizations in a Center for Medicare & Medicaid Innovation demonstration to reduce readmissions.
Following an introduction by Dr. Andy Miller (medical director of Healthcare Quality Strategies, Inc.), Marie Perillo, RN, BSN (VNA Health Group director of care coordination) will discuss the goals of transitional care and how the CNJCTP partnerships were formed. Then, Kimberly Mora, RN, BSN (VNA Health Group manager of care coordination) will describe how the care intervention model was developed. The presentation will wrap up with a real-life case study presented by Fiona McCaskie (geriatric social worker for Middlesex County Office of Aging & Disabled Services).
“We are excited to share where we started, lessons we have learned along the way, and where we are at now with improving population health through our CNJCTP project in New Jersey,” said Perillo.
