Wellframe Continues to Gain Momentum and is Named Winner of Merck | Heritage Provider Network Innovation Challenge
Boston, MA (PRWEB) May 19, 2014 -- Wellframe, a Boston-based company that combines mobile technology and artificial intelligence to amplify caring clinical relationships by engaging patients in personalized care plans, today announced that the organization has been named winner of the Merck | Heritage Provider Network Innovation Challenge. This award closely follows the dual announcements that the Wellframe had secured $1.5 million in seed funding this past April, and that the first commercial rollout of the Wellframe solution was completed in February.
As part of the Challenge, the Wellframe team conducted a pilot exercise in conjunction with Desert Oasis Healthcare, which is part of the Heritage Provider Network and provided a sophisticated managed care environment for this testing phase. Through the pilot, the team confirmed that frontline clinicians could utilize the Wellframe solution to engage with patients outside the walls of the hospital, and thereby improve both the efficiency and efficacy of the care provided.
Using Wellframe, each patient had a multimedia evidence based care plan created in partnership with frontline clinicians at Desert Oasis Hospital delivered to them through a custom app built on the Wellframe platform. Patients’ progress could be reviewed anytime, anywhere from the Wellframe dashboard to track progress, offer support and encouragement, or to highlight areas of concern.
Wellframe’s clinically proven proprietary method of delivering care plans to patients can be extended to any type of complex chronic disease care plan. Wellframe is has completed pilots with several academic medical centers across a number of clinical verticals and expects to make a number of customer announcements over the coming months.
