Scientists used to think that an infection with a drug resistant strain is relatively rare in those with a healthy immune system. However, recent research showed that HSV drug resistant strains are becoming more common in otherwise healthy people, especially when the virus infects “immune privileged” sites. These are locations in the body that are able to tolerate the introduction of antigens without eliciting an inflammatory immune response. For example, scientists wrote in a recent study that, drug resistant strains of herpes should be suspected in “cases of ACV unresponsive herpetic keratitis (a viral infection of the eye caused by the herpes simplex virus) and herpes simplex encephalitis (an infection of the brain with HSV) (2).” Specifically, the study authors wrote that “Surveillance of HSV drug-resistance is highly recommended in immunocompromised patients and in immunocompetent individuals with infections implicating 'immune-privileged sites' to rationally adapt antiviral treatment (2).”

If acyclovir, the standard treatment against HSV, proves ineffective, doctors may prescribe foscarnet, vidarabine, and cidofovir. However, these drugs showed only limited efficacy (3).

Under normal circumstances, and when fighting a non-drug resistant strain, “two types of antiviral treatments against HSV are available: topical and oral. The treatments include penciclovir, acyclovir, famciclovir, and valaciclovir. How- ever, their effectiveness is limited. For instance, a meta- analysis of five placebo-controlled and two dosecom- parison studies evaluated the effect of aciclovir, famciclovir or valaciclovir on symptoms.
