PopHealthCare announced today the acquisition of Educerus Health, a leading provider of clinically-driven in-home screening programs that focus on the evaluation of patients with comorbid risk factors for undiagnosed chronic conditions. In addition to the benefits associated with identifying disease burden earlier in the disease spectrum, the Educerus Health diagnostic suite targets and closes 12 different Quality/Star gaps in the home setting.

PopHealthCare currently provides field-based high-risk population care solutions and comprehensive risk adjustment services. The acquisition of Educerus Health allows PopHealthCare to expand its clinical reach by supplementing existing program capabilities with unique Educerus Health processes, tools and specially trained teams of clinicians. In addition to a comprehensive assessment platform, the Educerus Health program targets four of the most prevalent conditions for guideline-driven evaluation and diagnosis: chronic kidney disease (CKD), diabetes, vascular disease and obesity.

Educerus Health distinguishes its services from other in-home assessment vendors with this focus on at-risk patients, enabling the empirical diagnosis of conditions that are inconsistently evaluated and if left unidentified, can adversely impact quality of life for patients and increase costs to the healthcare system. These clinical insights are not possible through standard chart reviews or Health Risk Assessment (HRA)-style home assessments.
