PopHealthCare announced today that Elderplan, Inc. is expanding their long-term relationship by implementing CareSight, PopHealthCare’s high risk patient care services. The program will work closely with Elderplan’s physician network by supplementing in-office care with a comprehensive care management program, inclusive of house call visits to provide care in the homes of high risk members with chronic illness or who may have difficulty traveling to primary care regularly.
“We are excited to work with Elderplan, an organization that is clearly focused on providing high quality and innovative care to their membership - when and where they need it most,” said Mike Tudeen, Chief Executive Officer of PopHealthCare. “Our CareSight program fits perfectly with Elderplan’s goal of continuously innovating how they serve their membership.”
The CareSight program uses advanced analytics to predict those members most at risk for avoidable hospitalizations and complications due to their conditions and provides a comprehensive system of care with a mobile team of clinicians led by a physician, nurse practitioner, or physician’s assistant. Care is provided through in-home visits supplemented by telephonic outreach, 24-hour access, and other critical services such as improving medication adherence and integrating physical and behavioral interventions.
“Our partnership with PopHealthCare has been successful and we believe this expanded relationship will help us strengthen our already strong commitment to helping Medicare, Medicaid and Dual-Eligible adults at every level of health,” said Ronald M. Chaffin, President of Health Plans for Elderplan. “Together, we will focus on ways to improve health outcomes, as well as quality and member satisfaction by bringing an innovative in-home care delivery program directly to Elderplan members.”
