Elderplan, Inc. Expands Relationship With PopHealthCare Bringing In-Home Care to Medicare Members
Tempe, Ariz., and Brooklyn, N.Y. (PRWEB) July 18, 2014 -- PopHealthCare announced today that Elderplan, Inc. is expanding their long-term relationship by implementing CareSight, PopHealthCare’s high risk patient care services. The program will work closely with Elderplan’s physician network by supplementing in-office care with a comprehensive care management program, inclusive of house call visits to provide care in the homes of high risk members with chronic illness or who may have difficulty traveling to primary care regularly.

The CareSight program uses advanced analytics to predict those members most at risk for avoidable hospitalizations and complications due to their conditions and provides a comprehensive system of care with a mobile team of clinicians led by a physician, nurse practitioner, or physician’s assistant. Care is provided through in-home visits supplemented by telephonic outreach, 24-hour access, and other critical services such as improving medication adherence and integrating physical and behavioral interventions.
