HackensackUMC and MediconneX Announce Clinical Research Collaboration
Ramsey, NJ (PRWEB) July 21, 2014 -- This agreement will enable the joint development of technology and clinician-led programs to improve the transition of patients from the hospital. It has been recognized that poor transitions of care are often at the root of preventable readmissions leading to higher costs and lowering the quality-of-life for patients. The two organizations will be working together to leverage the clinical expertise of the HackensackUMC team through Mediconnex technology software to improve the care transitions for a broad range of patients.
As part of this collaboration, HackensackUMC and Mediconnex will be conducting - and expect to publish - studies showing the effectiveness of the improved transition program.
