HackensackUMC and MediconneX Announce Clinical Research Collaboration

We are very excited to announce the collaboration and research agreement that has been reached between HackensackUMC and MediconneX, LLC a developer of mobile health software.

This agreement will enable the joint development of technology and clinician-led programs to improve the transition of patients from the hospital. It has been recognized that poor transitions of care are often at the root of preventable readmissions leading to higher costs and lowering the quality-of-life for patients. The two organizations will be working together to leverage the clinical expertise of the HackensackUMC team through Mediconnex technology software to improve the care transitions for a broad range of patients.

As part of this collaboration, HackensackUMC and Mediconnex will be conducting - and expect to publish - studies showing the effectiveness of the improved transition program.

MediconneX, LLC
Mediconnex, a mHealth SaaS provider solution improves care transitions and reduces preventable readmissions by taking a patient centric approach in managing the discharge process. We have developed an easy to use discharge application that utilizes an alert and reminder system. This will allow clinicians to monitor compliance, including adherence to medications, a key patient safety issue. Our patent pending technology has the ability to help the hospital transform complex clinical data into more meaningful and accessible information for both clinicians and patients. Through our technology we are able to help patients successfully bridge the gap from hospital to home. Evidence shows that strong patient engagement, results in healthier outcomes. The post discharge follow up process is critical to a positive patient experience.
