“We believe if patients measure their blood pressure and heart rate every day at home, and take their medications as directed, there’s a segment of the CHF population who will see clinical benefits"
San Francisco, CA (PRWEB) September 08, 2014
Today, MyDigitalHealth Network, Inc. announced the introduction of the first free mobile platform for managing Congestive Heart Failure (CHF) at Techcrunch Disrupt. The platform (7-inch tablet) is pre-configured to each individual patient and incorporates a comprehensive suite of functions to significantly reduce avoidable and costly hospital readmissions.
Heart Failure affects 5.5 million people in the US and is the #1 reason people over 65 are readmitted to the hospital, at an estimated cost of $32 billion annually. “We’ve been working with some of the top Heart Failure specialists in the country to build an elegantly simple, yet robust product to allow patients to manage their chronic disease post-discharge,” says David Dubbs, Founder and CEO of MyDigitalHealth Network. “We believe our platform can dramatically improve the current rate of readmission – approximately 1 in 3 within 90 days, while allowing people to live comfortably at home.”
“We believe if patients measure their blood pressure and heart rate every day at home, and take their medications as directed, there’s a segment of the CHF population who will see clinical benefits,” says Arvind Bhimaraj, Director of Heart Failure Disease Management at The Debakey Heart & Vascular Center, Houston Methodist Hospital.
Centers for Medicare & Medicaid Services (CMS) has identified Heart Failure as one of 3 chronic diseases that will have penalties for hospitals when patients are readmitted within 30 days. “Our turnkey platform is free to hospitals and patients so we think the timing is prime for MyDigitalHealth Network,” says Dubbs. “In fact, CMS is implementing a Critical Care Management (CCM) program to reimburse doctors $42 per month per patient for non-face-to-face chronic care, so physicians can be paid for prescribing our platform to qualified patients.”
MyDigitalHealth Network will be in Start-Up Alley at Techcrunch Disrupt for people looking to get a hands-on demo.
