Centers for Medicare & Medicaid Services (CMS) has identified Heart Failure as one of 3 chronic diseases that will have penalties for hospitals when patients are readmitted within 30 days. “Our turnkey platform is free to hospitals and patients so we think the timing is prime for MyDigitalHealth Network,” says Dubbs. “In fact, CMS is implementing a Critical Care Management (CCM) program to reimburse doctors $42 per month per patient for non-face-to-face chronic care, so physicians can be paid for prescribing our platform to qualified patients.”
