Social Solutions Global, Inc., the leading provider of performance management software for human service organizations, today announced the recent release of its Choice Neighborhood Performance Models, designed specifically for Choice Neighborhoods planning and implementation grantees.

Performance Models are research-based configurations of Social Solutions’ performance management software platform, Efforts to Outcomes (ETO®) software, designed to support program performance management for frontline staff and managers in a particular type of program. Performance Models enable Choice Neighborhood grantees to rapidly deploy pre-configured but flexible models that can track highly individualized, locally-driven services at the participant level while reporting on common outcomes at the aggregate level, across multiple sites and programs.

The Choice Neighborhood Performance Model suite is the first of its kind and is composed of four models which support the various aspects of successfully implementing a Choice Neighborhood program:

  • Case Management: Supports evidence-based and sound practices in all the key components of case management, including assessments, service planning, intervention, coordination and resident surveys. Also support programs’ need to produce data that show progress across clients.
  • Housing: Allows communities to track all of their housing units in ETO and produces the federally required Housing Performance Measures report at the click of a button.
  • Neighborhood: Allows communities to record federally required neighborhood performance measures–collected from various sources–quarterly. The “Neighborhood Performance Measures” report can be generated directly from ETO, enabling communities to view change over time on key indicators of neighborhood improvements.
  • Community Engagement: Aids grantees in conducting comprehensive community needs assessments, tracking the extent to which local residents, businesses and other agencies are engaged, and managing partnerships between the lead agency, community organizations and other entities.
