Hospice of Michigan Partners with Enclara Health and Hospice Pharmacia to Provide Pharmacy Services for Hospice Patients

Hospice of Michigan returns to Hospice Pharmacia noting their ability, through their broad community pharmacy network, to meet the timely need of its hospice patients.
As Hospice of Michigan’s pharmacy service provider, Enclara and Hospice Pharmacia will provide Hospice of Michigan with a complete suite of pharmacy service tools, including 24/7 clinical pharmacy support for medication therapy consults, a comprehensive formulary with clinically appropriate and cost effective medications, and same-day and overnight mail-order medication delivery capability. Additionally, Hospice of Michigan will have access to combined company’s online platform and reporting tools to track patient outcomes, monitor medication utilization, and identify cost saving opportunities. Enclara and Hospice Pharmacia will also provide support to Hospice of Michigan with the Medicare Conditions of Participation (CoPs) regulations and reporting requirements.
