Solutions for Creating Healthier Populations

Today’s healthcare providers are faced with increasing reporting requirements and being in practice means more than just taking care of patients. Today, you also have to be able to manage all of your patients’ data, - Ryan Dickert

i2i Systems today announced it has been selected by Florida-based Meadowcrest Family Physicians to provide Population Health Management and Analytics solutions.

Meadowcrest Family Physicians is a private practice based in Crystal River, Florida. The father-son physician team serves about 2,500 patients and is growing rapidly.

Along this Gulf Coast town, many of those patients are on Medicare. In order to better manage their aging patients and take advantage of new Medicare rules that will soon pay physicians for managing patients with multiple chronic diseases, the physicians recognized the need for better access to patient data.

“Today’s healthcare providers are faced with increasing reporting requirements and being in practice means more than just taking care of patients. Today, you also have to be able to manage all of your patients’ data,” said Ryan Dickert, who will be joining his father and brother in the practice once he finishes medical school.

“Of course, we use an EHR, as well as other systems, and they contain all the data, but it’s just not easily accessible. i2iTracks gives us the ability to actually use all of the information in those systems and gives us a 30,000-foot view of the patient,” Ryan continued. “Now we spend less time looking for information. We’re more efficient and that allows us to spend more time with our patients.”

i2iTracks is the leading Population Health Management and Analytics solution, used by more than 1,000 healthcare organizations across 35 states. It unlocks and standardizes data from disparate electronic health records, practice management and other systems, simplifying the path to actionable information for an entire patient population. This empowers care teams to proactively, rather than reactively, manage patients leading to increased staff efficiency and better clinical outcomes.

“This country has been stagnant too long in the way we practice medicine. We don’t need big vertically integrated systems run by hospitals. Instead, our goal is to develop a strong, horizontally integrated outpatient system that keeps our patients healthy and out of the hospital,” said Dickert.

“Partnering with innovative private practice physicians like Meadowcrest means we have the ability to impact even more patients lives,” said Janice Nicholson, founder and CEO of i2i Systems. “Our solutions are able to help private practices achieve the quality measures that impact the health of their patients and the health of their practice.”
