eCaring Signs Care Management Contract with Elderplan, Inc.
New York, NY (PRWEB) September 23, 2014 -- eCaring, a leading health care quality company, announced today that it has entered into a contract to provide its unique Web-based home health care management and monitoring system to Elderplan, Inc., one of New York City’s largest not-for-profit health plans. Elderplan, an affiliated agency of MJHS, will deploy the eCaring system to help enhance the care of its Medicare, Medicaid and Dual-Eligible members.
“For nearly 30 years, Elderplan has been at the forefront of offering New Yorkers a range of innovative health plans that are designed to deliver positive health outcomes and member satisfaction,” said Eli S. Feldman, CEO of MJHS. “We selected eCaring to help Elderplan enhance the personalized high-quality care our members already receive at home and that we are so proud to offer. We also hope to reduce emergency room visits and hospital readmissions, which are as disruptive to our members as they are costly to the plan.”
eCaring’s transforming technology turns the home into a powerful real time source of actionable health care information that can capture and generate an average of 300 to 1,000 patient data points per month. Once implemented, the eCaring system will help empower Elderplan care managers by providing extensive amounts of behavioral, clinical and medication adherence data. Changes in critical vital signs, such as weight, blood sugar or blood pressure, as well as missed medications, changing sleep or eating patterns and other actionable information are filtered into alerts and notifications that can be quickly addressed by the care manager and Elderplan support team.
“We look forward to helping Elderplan members continue to get the best possible care,” said Robert M. Herzog, founder and CEO of eCaring. “Our award-winning icon-based mobile care management platform will also help dramatically reduce the overall cost of care, while streamlining the care management process, enhancing care management efficiency and improving overall member satisfaction.”
Elderplan joins eCaring’s growing list of clients, which includes Jewish Home Lifecare, Beth Israel Medical Center, Senior Health Partners, and others.
