Social Solutions Global, Inc. celebrates the CIE San Diego for launching Pilot, securing a second $1 million grant, and becoming a “Live Well San Diego” Partner.

San Diego CA, Dec. 10, 2014 — Social Solutions, the leading provider of performance management software for human and social services, is proud to celebrate their client, Community Information Exchange (CIE) San Diego, for accomplishing recent achievements. CIE has launched an innovative trust network that enhances care coordination serving their homeless population by sharing client-level information and they earned a second $1 million grant from the Alliance Healthcare Foundation which will go towards serving aging seniors in the community. They are also designated to become recognized as a “Live Well San Diego” partner on December 11.

CIE San Diego recently announced the go-live of a Pilot linking two organizations serving the homeless with the City of San Diego’s ambulance service and two hospitals, one of the first social service information exchanges. The CIE data network offers a more holistic view of a client’s social history and makes it easier for case managers and intake/discharge coordinators to connect with other care providers.

Because of the successful launch of the Pilot, CIE San Diego was recently awarded a second grant of $1 million to further expand their network to aging seniors in the community as well as measure organizational and client-level outcomes. The designation to become a “Live Well San Diego” (http://www.livewellsd.org/) partner reflects CIE San Diego’s dedication to supporting better service delivery systems and the Live Well goals of achieving a shared vision of building better health, living safely, and thriving for all San Diegans.

CIE uses Social Solutions’ Efforts to Outcomes (ETO®) software to collect manage and analyze program data. They have configured ETO software to have multiple API connections as well as a custom enterprise Quick Search feature.

“By allowing access to new client information, CIE San Diego’s sharing network technology and processes enable participants to improve their clients’ lives, enhance operational effectiveness, and design new service delivery models,” said Scott Bechtler-Levin, Executive Director of CIE San Diego.

Community Information Exchange San Diego (http://www.CIESanDiego.org) is an innovative nonprofit and collaboration catalyst that works with a wide range of partners in San Diego County.    CIE San Diego aims to strengthen the non-profit sector by creating a comprehensive social service information exchange that will facilitate inter-agency client care coordination and transform program collaboration across thousands of diverse participating organizations. Start-up funding has been provided by the Alliance Healthcare Foundation.
