Health Dialog Receives NCQA Accreditation for Disease Management Programs
Boston, MA (PRWEB) December 15, 2014 -- Health Dialog, a leading provider of population health management solutions, announced today that it has received Patient Oriented Accreditation from the National Committee for Quality Assurance (NCQA) for its Disease Management (DM) Programs for asthma, congestive heart failure, chronic obstructive pulmonary disease (COPD), coronary heart disease and diabetes. This marks the 12th continuous year of accreditation from NCQA for Health Dialog’s Disease Management (DM) Programs, reaffirming Health Dialog’s focus and strength in facilitating positive patient outcomes and high-quality care.

NCQA awards its highest status of Accreditation to organizations that meet or exceed its rigorous standards and guidelines. The evaluation process is intended to help organizations achieve the highest possible level of performance, increase adherence to care guidelines and create an environment of continuous improvement. NCQA’s standardized performance measures address dimensions of patient care for each DM program and are organized into seven categories: Evidence-Based Programs, Patient Service, Practitioner Service, Care Coordination, Measurement and Quality Improvement, Program Operation and Reporting DM Performance. Health Dialog’s accreditation extends from November 19, 2014 to November 19, 2017.
