Summit Healthcare CCD Solution Helps Porter Medical Center Enhance Patient Care
Braintree, Massachusetts (PRWEB) December 22, 2014 -- Porter Medical Center needed a Health Information System Provider (HISP) whose continuity of care document (CCD) solution would help meet Meaningful Use Stage 2 direct messaging requirements. To achieve this goal, the organization chose Summit Care Exchange as the secure data exchange platform to enable communication across its community health information exchange (HIE) network.

Summit Care Exchange enables Porter Medical Center to send patient clinical summaries to the electronic mailboxes of more than 60 physicians, all who currently leverage the Meditech Ambulatory EHR system. By ensuring that physicians receive CCDs in a timely manner, Summit Care Exchange supports smooth transitions of care and optimal decision making throughout each patient’s visit. The solution also enables direct messaging to physicians at Porter Medical Center’s 12 physician practices across its HIE.

Additionally, they will expand their investment in direct messaging to further correspond with the community to include additional report types such as Lab, Radiology and Transcription.
