Casenet®, LLC, a leading provider of extensible care management solutions, today announced the release of its TruCare® 5.3 care management platform with major enhancements to its user interface, assessments, care plans, correspondence module and mobile capabilities. The enhancements in the latest release are designed to simplify the complexities of health management by automating workflow to decrease the time to intervention, increase efficiency and member engagement, and improve care quality. By helping dynamic, multidisciplinary care teams better coordinate, collaborate and communicate, TruCare delivers more integrated health management, which in turn supports individuals in achieving their maximum health.

The TruCare 5.3 enhancements will improve user efficiency and accuracy for care teams at health plans, state and government organizations, ACOs, TPAs and other risk-bearing entities. Care teams will be able to more quickly evaluate and step in to help members and to enhance member engagement. Improved care plans, correspondence and mobile capabilities will allow care teams to better target and care for their members regardless of location and more accurately tailor communications and individual service plans (ISPs). Additionally, TruCare’s enhanced integration and Web services capabilities will enable clients to seamlessly share information between TruCare and different clinical, financial and administrative systems. Continued enhancements to the TruCare platform are driving results for Casenet clients with 13 Casenet clients achieving NCQA accreditation status since moving to TruCare, and five achieving NCQA commendable status.

While many enhancements were made in TruCare 5.3, highlights include:

  •     Redesigned user interface screens and assessment workflow to increase system and user efficiency
  •     Enhanced assessment conditional logic to improve member-centric information collection and to reduce member health risks
  •     Implemented new correspondence templates using Microsoft Word technology to support complex formatting and language needs
  •     Evolved mobile capabilities to support care members in the field and decrease time to intervention, improve care quality and reduce care costs
  •     Added new data feeds and Web services to increase interoperability and enable a more complete view of a member’s health
  •     Enabled new rules processing to identify and target members who are at risk for adverse events and allow for more rapid intervention and less costly treatment options
  •     Updated reporting capabilities to ensure quality levels are being met and to optimize care delivery
