Pittsburgh, PA (PRWEB) January 20, 2015
A Pittsburgh healthcare services company today announced an innovative program that reduces healthcare costs while helping save lives.
AdvaCare Home Services, Inc. launched the Patient Partner Program to help hospitals and practices meet the goals and needs of patients in this new era of healthcare.
“By becoming and advocate for both patients and physicians, AdvaCare has found a way to bridge treatment for patients from hospital to home and help reduce overall healthcare costs,” AdvaCare Home Services President Tammy Zelenko said.
The Patient Partner Program was created on a foundation of trust and patient advocacy, with five pillars in place supporting the program. The pillars are customized clinical care, patient advocacy, clinical respiratory programs, technology and innovation, and outcomes and reporting.
By becoming the eyes and ears in the field for physicians and hospitals, the Patient Partner Program is able to bridge the gap in care in two ways. The first is through continuous monitoring of patients utilizing cutting-edge technology, and the second is through detailed, individualized patient care plans and in-home assessments.
The program aims to help reduce the overall healthcare costs for patients by reducing the number of readmissions, emergency room visits, and additional health complications that can occur during the hospital-to-home transition. It is accountable with lowering hospital readmission penalties while providing better outcomes for patients.
“The program offers physicians and doctors an additional route that helps save lives and money,” Zelenko said. “The focus is not only to help patients adjust to their diagnoses, but to make life-style changes, and educate them on their chronic diseases. Through this level of dedication, AdvaCare is helping hospitals avoid costly readmission penalties allowing more involved patients to better manage their healthcare,” she added
One example of how the Patient Partner Program works is when a patient is referred to AdvaCare Home Services for oxygen needs they are enrolled in the AdvaCare Advantage program. The program gives the patient respiratory clinical support during the critical 30-day post-discharge period through multiple contacts with a respiratory therapist in the home and on the phone. This ensures that the patient is maintaining their treatment plan and progressing toward their goals. Patients are also evaluated with overnight oximetry and physicians are quickly alerted to any clinical concerns and actions by the respiratory staff.
AdvaCare Home Services is a locally owned home medical equipment and respiratory company that has been in the durable medical equipment field since 1996. It is a leading provider of many services such as oxygen for home or travel, sleep therapy, ambulatory, and home safety equipment.
AdvaCare’s Patient Partner Program is innovative patient-centered care that is clinically proven to reduce and control healthcare costs, one patient at a time. The Patient Partner Program provides a level of unprecedented care and results in cost savings by engaging patients through education, assessment and coaching.
