Chase Brexton Selects i2i Systems Population Health Management Solutions to Demonstrate HEDIS Quality Outcomes to Maryland’s Health Plans

Chase Brexton provides comprehensive primary care, behavioral health and dental services to more than 30,000 patients across seven clinics in the greater Baltimore area of Maryland. As early adopters of electronic medical records, the organization has always focused on providing quality care.
According to Richard Larison, Chase Brexton’s CEO, while they were good at putting information into the EMR, they were often unable to get the necessary information out of the EMR to demonstrate quality outcomes.

To do this, Chase Brexton implemented i2iTracks across all seven locations and reengineered the way they operate. “Every patient who comes in is assessed as to their current health status,” said Larison. “Then we work as a team to improve them to their highest possible health status.
“We have implemented new protocols and procedures and we work to engage patients in their health. We are able to give feedback to our provider teams in meaningful ways that help them make decisions and change what isn’t working,” Larison continued.
The Morning Huddle report in i2iTracks was one of the starting points for changing the way the organization delivered care. “The Morning Huddle got the care teams talking proactively about the patients who were about to come in. We were able to identify and eliminate gaps in care and implement better care plans for our patients in order to achieve the desired outcomes,” Larison explained.
In joint-partnership with the Mid-Atlantic Association of Community Health Centers, Chase Brexton and a network of other Federally Qualified Health Centers in Maryland purchased PopIQ, the cloud-based global health analytics solution from i2i Systems.
PopIQ aggregates de-identified patient data across multiple organizations and allows for comparative analysis. The goal of implementing PopIQ across Mid-Atlantic Association of Community Health Centers is to support HRSA and BPHC requirements, as well as meet the health outcomes goals of the state and local government and local health plans.
