Dr. David Hanekom Presenting at Annual Osteopathic Convention
Alpharetta, GA (PRWEB) March 12, 2015 -- Population health expert, David Hanekom, M.D, F.A.C.P., C.M.P.E., will present at the 52nd ACOFP Annual Convention and Scientific Seminars taking place March 11 through 15 in Las Vegas, Nevada. Dr. Hanekom is the Chief Medical Officer at Symphony Performance Health where he oversees population health initiatives.
Dr. Hanekom will collaborate with Eastland, Texas osteopathic family physician Robert DeLuca, DO, FACOFP, a Governor on the ACOFP Board of Governors and Past President of the Texas Society of the ACOFP.
Their continuing medical education discussion, “The Future of Patient Data Reporting and Population Health Management”, will be held from 2:00 p.m. until 3:00 p.m., Thursday, March 12. The discussion cites the June 2013 article, “Team-Based Care: A Critical Element of Primary Care Practice Transformation” published by G. Goldberg, T. Beeson, A Kuzel, L. Love, and M. Carver, which provides an understanding of patient-centered care and finds that team-based care models show success in transforming primary care practices.
Since his graduation from the University of the Witwatersrand, in Johannesburg, South Africa, Dr. Hanekom has studied family and internal medicine at the Medical University of Southern Africa as well at the University of North Dakota School of Medicine and Health Sciences. He is a Fellow of the American College of Physicians and previously a Clinical Associate Professor of Medicine at the UND School of Medicine and Health Sciences. As a previous Chief Medical Officer at a nationally branded health plan and Director of Chronic Disease Management at Sanford Health System North, Dr. Hanekom has gained expertise in population health management and strategies which improve healthcare outcomes.
Dr. Hanekom guides development of population health analysis products for Symphony Performance Health (SPH).
