Exari Streamlines Patient Plan Records for Esdégé-Reigersdaal

Exari worked closely with Hemington Consultants for project management and Tier2 Consulting for detail design, build and integration.
Esdégé-Reigersdaal ([http://www.esdege-reigersdaal.nl/) is an organization providing healthcare services to people with disabilities. They provide support for children, youth and adults with physical and/or mental disabilities or acquired brain injury.
The solution allows Esdégé-Reigersdaal employees to easily create, manage and access patient plan records. These include intake, care plans, activity and progress reports, test results, interviews and videos. As a result, the process is streamlined and patient care is improved.

Using the Esdégé-Reigersdaal Electronic Client Dossier system, healthcare staff now have access to up-to-date information on over 2,000 patients in configurable dossiers, which use Exari’s technology to store and update patient data. Exari’s dynamic document templates are configured for easy assembly of crucial forms, such as Support Plans (OSP) through patient interviews. The system can also report, access and create daily goal reports based on mental, physical or emotional states, enabling long-term monitoring of patient progress.
All information on the platform is kept secure and compliant with the Wet Bescherming Persoonsgegevens (data protection) regulations through a role-based security model, which supports the authentication matrix required by Esdégé-Reigersdaal.
