Memorial Hospital Selects MD Revolution's Digital Health Platform to Deliver a New Form of Chronic Care Management
(PRWEB) May 27, 2015 -- SAN DIEGO, May 27, 2015 – Memorial Hospital, based in Gulfport Mississippi, is the first healthcare provider along the Gulf Coast to pioneer a new approach to chronic care management (CCM). In partnership with MD Revolution, a digital care management provider, Memorial is introducing RevUp, a new program that enables healthcare providers to easily meet Medicare’s CCM requirements and increase monthly reimbursements. This new, user-friendly program is designed to improve patient engagement, reduce hospital readmissions and increase healthcare providers’ Medicare reimbursements.
The Medicare-covered program, RevUp, is the first digital health platform to help patients and healthcare providers engage in between office visits, empowering patients to take a more active role in their health care. With RevUp, Medicare patients with two or more chronic conditions will have access to a registered nurse/health coach to discuss their health goals and assist with medication monitoring.

In a previous study with Sharp Healthcare, patients on RevUp achieved excellent clinical outcomes, including:
• 70% of patients experienced weight loss and were able to change their body composition profile, losing dangerous visceral fat;
• 80% improved their cardiorespiratory fitness;
• More than 70% remained engaged with the program six months after the study concluded.
