Florida Hospital Physician Group (FHPG) and Florida Hospital, members of Adventist Health System, have selected Smartlink Mobile Systems to meet the Centers for Medicare and Medicaid Services (CMS) electronic requirements for Chronic Care Management (CCM) services. Florida Hospital is among the first large hospital systems to participate in the new CMS CCM program, aimed at improving the health and wellness of Medicare beneficiaries with two or more chronic conditions.
