Rise Health Platform Supports Comprehensive Primary Care Initiative in Connecting Colorado
Denver, Colorado (PRWEB) July 17, 2015 -- Colorado primary care providers are experiencing for the first time the real benefits of multi-payer collaboration in an effort to transform healthcare across the state. The Colorado CPCi Multi-Payer HIE Collaboration, as the case study is titled, showcases the value of leveraging multi-payer data in coordination with Quality Health Network and CORHIO in order to:
• support payers and providers in improving on existing care coordination activities and better measuring impact
• allow practices to identify patients through rapid registry development
• and enable targeted care management through the delivery of real-time care alerts to improve quality, lower costs and personalize the care experience for patients
Jeff Harner, Chief Analytics Officer for Rise, will be joined by Marc Lassaux, CTO of Quality Health Network (QHN) and Dr. Mary Reeves from 1st Street Family Health, in Salida Colorado to share details about the program they’ve developed as part of the Colorado CPCi at 2:20 MT on July 21st at the Sheraton Downtown.
