LeadingAge Selects Eldermark Case Study to Highlight Medication Error Reduction Results
Minneapolis, Minn. (PRWEB) July 31, 2015 -- Eldermark Software, the innovative leader of senior care software, is selected as a contributor for LeadingAge’s Center for Aging Services Technologies (CAST) Medication Management Technologies whitepaper. The whitepaper is a compilation of three case studies designed to help long-term and post-acute care providers understand the benefits of using medication management technologies.
Eldermark’s case study is in partnership with Epiphany Assisted Living, a non-profit housing-with-services provider. The project focused on the implementation of the Electronic Medication Administration Record (eMAR) within the Electronic Health Record (EHR) of its licensed home care program. The goal of the study is to demonstrate a reduction in medication error rates by using eMAR verses a standard MAR system.
The case study results for Epiphany Assisted Living are:
• Reduced medication error rate from .072% in 2013 to .011% in 2014
• Achieved a more complete EHR for each resident
• Reduced month-end reconciliation workflow by 10 hours per nurse
• Enhanced regulatory compliance to audit medication administration
• Enhanced caregiver accountability with missed medication/treatment reporting
• Increased resident/workforce safety
• Mitigation of drug diversion through dashboard alerts

In addition to CAST whitepaper, Eldermark was also chosen to present its case study at the Long Term and Post-Acute Care HIT Summit in Baltimore on June 23, 2015. The Epiphany Assisted Living case study was the only one to be presented.
