Avamere Rehabilitation of Hillsboro Receives Perfect Oregon Department of Health Services Inspection Results

The unannounced inspection, which is conducted annually by DHS in accordance with strict guidelines administered by the Centers for Medicare & Medicaid Services, examined patient quality of care and quality of life, dietary services, fire safety, and environmental services.

No deficiencies were found at Avamere Rehabilitation of Hillsboro, which is the first established and longest operating skilled nursing facility for the Avamere Family of Companies, during the entire State inspection period. The perfect survey score is the best score a skilled nursing center can receive and is a merit achieved by only a small percentage of skilled nursing facilities in the state. The survey process also included interviews with patients, patients’ family members and Avamere Rehabilitation of Hillsboro employees, and a record review of documentation.
