Oculus Health, the premier Chronic Care Management solution, has announced the addition of disease specific management modules to its patient care platform. These condition specific modules will facilitate the most targeted and effective management of chronic conditions such as diabetes, heart disease, hypertension, asthma, and stroke. Oculus believes this is the next great advancement in data driven care, specifically Chronic Care Management.

The product, Oculus Trackers™, delivers an evidence based individualized care plan for each condition, and assists providers with customizing patient care plans. According to Chief Medical Officer, Kishori Somyreddy, M.D., this will ensure that the most proven disease mechanisms are effectively delivered to patients, and that they are able to follow and report their care plan adherence in a consistent manner. Dr. Somyreddy led the team of clinicians in the development of the product.

"Oculus Trackers is one of the most exciting advances in bringing data to medicine that I've seen in my career," said Dr. Somyreddy. "It will facilitate a physician designed plan for each patient, customized to their needs and at the same time incentivizes the patient to manage the condition providing a sense of wellbeing and good health overall."

Recent data shows that more than 145 million people, or almost half of all Americans, live with a chronic condition. That number is projected to increase by more than one percent per year by 2030, resulting in an estimated chronically ill population of 171 million.

Nearly half of all people living with a chronic condition have two or more ailments. As a result, many managed care and integrated delivery systems have taken a great interest in correcting the many deficiencies in current management of diseases such as diabetes, heart disease, depression, and asthma.

"These deficiencies will require nothing less than a transformation of healthcare, from a system that is essentially reactive - responding mainly when a person is sick - to one that is proactive and focused on keeping a person as healthy as possible. Our tools help physicians to provide proactive measures and nudge patients to follow them," said Dr. Somyreddy.
