XchangePoint, Version 4.0, features a medication reconciliation solution that supports a pharmacist-led bedside consultation of medications at discharge. The result is an accurate, up-to-date discharge medication list that includes both home and newly prescribed medications.
By using an electronic medical record (EMR) or e-prescribing software certified to connect to the Surescripts network, hospitals or practices can reduce costs, enhance patient safety and improve practice efficiency. Medication history services allow prescribers to access critically important information on their patient’s current and past medications from pharmacy benefit managers and community pharmacies. This helps inform prescribers about potential medication issues.
