Mount Sinai Health System and Livongo Health Launch Innovative Effort to Better Manage Diabetes
(PRWEB) October 26, 2015 -- The Mount Sinai Health System and Livongo Health™, a consumer digital health company, today announced the launch of a comprehensive diabetes management program in the New York market. The program, already available to Mount Sinai employees, combines the Livongo for Diabetes solution with the clinical capabilities of the Mount Sinai Clinical Diabetes Institute and the Mount Sinai’s Diabetes and Cardiovascular Alliance to offer a comprehensive health management solution to patients with diabetes. The program will be expanded to broader populations across the New York market through Mount Sinai’s network of physicians and affiliated providers.

The program provides an end-to-end ecosystem that empowers people with chronic conditions, including diabetes, to live better. It starts with a smart data analytics platform that captures information from a variety of sources including Livongo’s connected glucose meters, personal devices, electronic health records, and medication records. Relevant and timely insights and alerts are then provided in real-time to the person with diabetes, their network of family and friends, and their Mount Sinai care team.
